F2 Represents shares of restricted stock granted to the reporting person pursuant to the Sanara MedTech Inc. 2014 Omnibus Long-Term Incentive Plan, as amended and restated. The shares of restricted stock will vest on the date of the Company's 2022 annual meeting of shareholders; provided that the reporting person is providing services to the Company until such date.
F3 Represents shares of the Company's common stock owned of record by CGI Cellerate RX, LLC, which is a wholly owned subsidiary of Catalyst Rochal, LLC. Mr. Nixon is a manager of Catalyst Rochal, LLC.
